Juniors Application

Provide the full name and postcode of the venue that the club will usually run home matches for.
Provide the name and main team colour of Team 1 'e.g. "Dorchester Dragons A - White" or "U10s - Grey"
Each team must declare a nominated coach. This can be a player/coach for Seniors league e.g. "Fred Bloggs", or a BE L2 qualified coach for Juniors e.g. "Ben Jones - BE L2"

For each of the team coaches declared above, please upload their 'Basketball England L2 Coach' certificates. You can upload more than 1 file (PDF/JPG)

Each club must provide one trained and competent Club First Aider who is responsible for player, member, supporter and visitor welfare during any fixture or event they are involved in. Please upload their First Aid certificates. You can upload more than 1 file (PDF/JPG)
Preferred Game Day and Tip-Off time for Team 1:
Please provide any additional supporting information which you feel will help your club and team(s) application join the DBA. For example the length of time your club has run for, any previous memberships your club has had to other leagues, any friendly matches played recently. If none state 'N/A'.
* From 24/25 season, Basketball England now require all new 'Safeguarding' course certificates to be BE issued course certificates by 31st August. Those with existing 'Safeguarding' course certificates remain valid until their end date as stated on the BE portal. Additionally, the nominated Club Welfare Officer needs to complete the 'Club Welfare Officer Safeguarding' course AND the new BE 'Safeguarding' course also by 31st August. Terms and Conditions

We confirm that the 'Home Venue' court will have availability to be booked for a sufficient amount of time to run all games for each team on game days as far as reasonably possible.

We confirm that it is not possible to change kit colours during the season without permission of the executive committee and will ensure all kits are compliant with Basketball England rules.

We confirm that we will provide competent table officials who are BE members, available to officiate the table at all home games. The club can also provide the technical equipment as set in article 40 of the Basketball England rule book.

We confirm that for the teams above, we can provide Basketball England player licenses to evidence that they are active club players. Note that fixtures will NOT include these teams/players until such evidence is presented by 31st August prior to the upcoming season.

and the DBA player affiliation fee has been paid.

We confirm that by filling in this form and submitting our application, all DBA league entry fees ( tbc usually £60.00) plus a holding deposit (£50.00*) are now due for the teams above. Note that fixtures will NOT include these teams until payment is received by 31st August prior to the upcoming season. If you already have a deposit with the league this may be rolled over from previous season.

Clubs also confirm they will provide referees to cover games across the junior section between 30-45 per season.
